When I saw "modem firmware" I had a flashback of at&f1
Anyways try http://192.168.1.1 or http://192.168.0.1. Are you trying to connect to the router or the modem? My fidelity modem actually does have a web based configuration page, but since you are trying to forward ports I'm assuming you meant router.
You might want to try a hard reset of the router, and then go back to the defaults from the documentation. It is possible (and easy) to change the router's IP.

with my router I had to turn off my firewall (I think I had zone alarm at the time) to access the configuration screen properly. After that it would properly display and allow me to forward ports. Don't know if that will help, but thus is the end of my dealings with routers aside from programming cisco ones.

Are you with Sprint/Embarq and on a 660 DSL router/modem?
